The real cost of AI governance failures

Now that you've assessed your program's maturity, let's look at what can happen when governance is missing or not working quite as intended. When AI is adopted without oversight, even small gaps can lead to big operational, financial, and reputational problems. A 2025 IBM report found that only about one-third of organizations conduct regular audits for AI misuse. An E&Y report found that nearly every large company who deployed AI experienced some type of financial loss. These were due to compliance failures, flawed outputs, bias, or disruptions to sustainability goals. Let's focus on three common areas, bias and fairness issues, poor vendor oversight, and lack of organizational control over AI. First up, bias and fairness. Imagine one company implemented an AI hiring tool to screen resumes.
